Heartland Alliance International in Nigeria works at the intersection of health and human rights, to provide HIV prevention and human rights protection services to most at-risk populations (MARPs), by sensitizing and building grassroots support organizations in the country. We seek qualified candidates to fill the listed position. Each candidate must meet the minimum requirements listed in the advert and should be committed to the promotion of human rights, regardless of age, ethnicity, class, or gender. Candidate must be willing to work with diverse population in a supportive and accepting manner.

ESSENTIAL DUTIES AND RESPONBILITIES

1. Ensures on-going tracking and support to program participants by maintaining contact by way of in person drop-in, email, telephone calls, and accompaniments.
2. Inform and educate the larger community members about HIV testing sites and the value of testing.
3. Provide basic HIV101 information at outreach events, health fairs, community gatherings and at the OSS.
4. Assess member knowledge and self-management skill level in the areas of HIV disease including treatment, preventing disease progression, preventing transmission, self-care, emotional wellness, social support, disclosure, and harm-reduction.
5. Help participants to develop self-management skills based on participant-identified needs and Clinic.
6. Refer members to HIV related health and wellness services and/or other community services.
7. Attend patient consults at request of clinic or patient.
8. In consultation with a Supervisor, promote the Peer Navigation Program at client tracking meeting.
9. Maintain confidential and accurate records and reports of all client tracking activities.
10. Comply with agency policies and procedures, including but not limited to maintaining confidentiality of participants and business operations in accordance with HAI policies.
11. Other duties as may be assigned.

MINIMUM REQUIREMENTS

1. ND/HND/B.Sc. in relevant field
2. Experience in HIV related health and wellness services, health advocacy and/or other community services is an added advantage.
3. Effective Communication and Problem solving skills.
