Jhpiego, an Affiliate of Johns Hopkins University is a global leader in improving healthcare services for women and their families. The TMEC/RISE program is a newly-awarded 5-year, USAID-funded global HIV program that supports countries to achieve and maintain HIV epidemic control through provision of strategic technical assistance and direct service delivery to improve prevention, HIV case finding and treatment programming. In Nigeria, TMEC/RISE will support the Government of Nigeria to address barriers in HIV services among at-risk adult men, women and priority populations and to build capacity of local implementing partners, continuously leveraging data to successfully course correct and achieve and maintain epidemic control. This project aims to achieve and maintain the primary prevention and 95-95-95 goals that are critical to HIV epidemic control in Nigeria. The program will also support PEPFAR’s transition of HIV service delivery to local partners, advancing progress along the journey to selfreliance. Overview:

Working under the guidance and direction of the Pharmacy & Supply Chain Officer for the RISE project in Nigeria, the Pharmacy Services Optimizer will support the facilities and the tactical epidemic control teams by ensuring availability of ARVs/OIs for all newly initiated and refill clients at SDPs. He/She will also co-ordinate aspects of pharmaceutical care that pertains to adherence counselling, ADR monitoring, retention and other pharmacy and supply chain component of the RISE project while working closely with the supported facilities and other SDPs in the LGA. S/he will actively support MMD uptake, regimen optimization, TPT start/completion and Pharmacy/Supply chain systems strengthening activities as well as quality improvement practices management at assigned supported Health Facilities.

Responsibilities

  • The Pharmacist will function as a vital member of the Tactical Epidemic Control teams as required; by ensuring availability of ARVs/OIs for all newly initiated and refill clients at the community and facilitate documentation/ integration into the facility Pharmacy daily worksheet.
  • Provide technical support in strengthening pharmacy systems and building Pharmacists capacity on pharmacy best practices in the area of PMTCT, ART and TB Care.
  • S/he will facilitate and coordinate the implementation of the differentiated service delivery

(DSD) models such as Community Pharmacy ARV Refill Program (CPARP), Community ARV Refill Clubs (CARC), Multi-Month Dispensing (MMD), Fast track refills and Appointment spacing.

  • Implement, coordinate and participate in Drug and Therapeutic Committees (DTC) meeting, facilitate the commencement of virally unsuppressed clients on Enhanced Adherence Counselling (EAC), monitor the switching of eligible clients to 2nd line regimen as well as ensuring proper documentation of these activities.
  • Coordination of Pharmacovigilance activities (adverse drug reaction monitoring and reporting using NAFDAC yellow forms).
  • S/he will manage the availability and promote rational use of ARV drugs and other essential medicines and ensure the use of recommended ART regimen across all age bands.
  • S/He will support the Pharmacy Focal Person in generation & report of high-quality Combined Reports Requisition Form (CRRF) and Patient Per Regimen (PPR) for ARVs/OIs bimonthly orders to ensure continuous availability of ARVs and OIs drugs for the project.
  • Monitor Stock Status of ARVs/OIs drugs and INH across supported facilities and as well support inventory management and control to avoid expiries and stock out of commodities.
  • Facilitate inter/intra facility redistribution of ARVs/OIs from facility with optimum stock to facilities in dire need as informed by the supply chain officer for the state.
  • Coordinate clients’ appointment scheduling, generation of defaulters’ list, tracking of daily and weekly ARV refills to ensure retention of clients in care.
  • Implement robust patient counseling, medication adherence and follow up monitoring using the 28 days adherence calendar.
  • Implement provision of drug information and health education to patients and other members of the health team.
  • Monitor and ensure appropriate storage conditions of ARVs and other commodities are met both at the facility and community.
  • Ensure availability and use of pharmacy job Aids, dosing wheels and dispensing IEC materials.
  • Implement the necessary measures for reverse logistics and appropriate regulation of waste

i.e. expired, damaged and phased out regimen from usable medicines and other essential supplies.

  • The Pharmacist will support and monitor documentation of Pharmacy services into data capturing/inventory control tools. This include combined pharmacy daily worksheet, appointment diary, IPT register, EAC register.
  • S/he will be involved in daily data triangulation with the LAMIS DECs for every pharmacy indicator including ART STAT, New on MMD, TPT, Pediatric regimen substitution etc.
  • S/he will Collate, Validate and Analyze data supporting project targets and indicator attainment from the facility and submits monthly reports like Monthly Stock Status Tracker (MSST), Pharmacy Services Monthly Summary Forms etc
  • To support tracking of mother baby pair by ensuring same day appointments for mothers and their children.
  • Performs other duties as assigned.

Required Qualifications:

  • Degree in Pharmacy (B. Pharm)
  • Experience in Logistics and Supply Chain Management and ART Pharmacy is an added advantage
  • Possession of an android device (Phone &/tablet) is mandatory • Knowledge of Computer (MS Word/Excel) is also an added advantage
