Pro-Health International is a faith-based, non-profit organization that has provided free healthcare services to under-served populations in Nigeria for over 31 years. CDC and USAID have funded PHI to implement HIV/AIDS prevention, treatment, and care projects, OVC projects, and Global Health security projects.

The ICHSSA-4 project is a 5-year USAID-funded award that commenced in December 2019 and will run up until December 2024. The project is being implemented by a consortium of partners, with Pro-Health International (PHI) as the lead partner and Catholic Relief Services (CRS) as the sub in Adamawa, Bauchi, Taraba, Sokoto, Kebbi and Zamfara states. The ICHSSA-4 project aims to ensure that Orphans and Vulnerable Children (OVC) are cared for and protected by their households, communities, local and state governments to reduce their vulnerability to HIV.

We are recruiting to fill the position below:

Job Description
Case Monitors (Responsible for the Facility-Community Bi-directional Linkages and Directly Supervised by the ICHSSA 4 Staff)

Case Monitors are solely responsible for:

  • Technical and programmatic leadership in the implementation of OVC interventions across the facility – community spectrum.
  • Supervise and give technical oversight to all cross-cutting linkage interventions by facility case managers and community case workers.
  • S/he manages and coordinates case conference among case workers to address concerns and issues arising from service provisions
  • Conduct initial comprehensive assessment of clients’ needs and support systems using the National OVC service standards and assessment tools
  • Supports the training of peer navigators, case workers on the care process for project beneficiaries
  • The CM is expected to strengthen community and health facility linkages and bidirectional referrals to optimize care, treatment and support services to vulnerable children infected or affected by HIV.
  • The CM will support training of case workers on gender norms, adolescent, nutrition, child protection programming.
  • The CM will interface on all spectrum of care for project beneficiaries and provide community level support to them including early childhood and parenting services.
  • Collaborate with Facility ART team to ensure CALHIV/ALHIV and PLHIV (Caregiver) is on optimized regiment and retained on treatment.
  • The CCM will provide support in the identification, linkages for Adolescent girls, women for support in SVAC and GBV response systems in the state.
  • CCM will participate in monthly case conferencing with PHI and conduct same with case workers at the CSO level.
  • CCM will collate, coordinate monthly reports of activities across the different thematic areas support, provide a summary of areas of challenges and analysis aimed at providing next line of interventions.

Primary Responsibilities

  • Provide assisted referral services and linkage to care and treatment services for test indicated children.
  • Supports adolescent OTZ clubs, adolescent programs, and community-based programming platforms such as Kids Clubs, Caregivers’ Forum that are usually facilitated by Case Workers.
  • Collaborate with facility case finding team to ensure newly identified HIV positive clients are properly linked to treatment.
  • Support research efforts including assessments using instruments developed for information gathering on the ICHSSA 4 project.
  • Collaborate with Facility ART team to ensure CALHIV/ALHIV and CA/PLHIV (Caregiver) are on optimized regimen and retained on treatment.
  • Develop and roll out mentoring, coaching and supportive supervision program for community case workers on service delivery to key population
  • Support the updating of viral load result through collaboration with health facilities staff
  • Monitor the implementation of care plans for each household in their community to ascertain the progress being made and the level of support being provided by the Community Case Workers.
  • Strengthen pediatric ART Saturation Support (PASS) at households and community levels in collaboration with the Peer Navigators.
  • Perform any other functions that might be assigned from time to time depending on emerging needs and priorities of the project.

Qualifications

  • Candidates should possess an HND / B.Sc Degree with 1 – 3 years relevant work experience.
