Jhpiego, an Affiliate of Johns Hopkins University is a global leader in improving healthcare services for women and their families. The TMEC/RISE program is a newly-awarded 5-year, USAID-funded global HIV program that supports countries to achieve and maintain HIV epidemic control through provision of strategic technical assistance and direct service delivery to improve prevention, HIV case finding and treatment programming. In Nigeria, TMEC/RISE will support the Government of Nigeria to address barriers in HIV services among at-risk adult men, women and priority populations and to build capacity of local implementing partners, continuously leveraging data to successfully course correct and achieve and maintain epidemic control. This project aims to achieve and maintain the primary prevention and 95-95-95 goals that are critical to HIV epidemic control in Nigeria. The program will also support PEPFAR’s transition of HIV service delivery to local partners, advancing progress along the journey to selfreliance. Jhpiego hereby invites applications from highly resourceful, experienced and dynamic professionals for the following:

Overview:

Reaching Impact, Saturation and Epidemic Control (TMEC-RISE) is a project funded by the United States Agency for International Development (USAID) and implemented by a consortium of partners led by Jhpiego. RISE project will collaborate with the Adamawa State Ministry of Health and other Implementing partners to achieve a shared vision of attaining and maintaining HIV epidemic control through sustainable, self-reliant and resilient health systems by 2024

Responsibilities

  • Provide treatment education, adherence counselling and peer support to assigned clients at health facilities and community following the adherence support schedule.
  • Facilitate active referrals of clients to treatment through escort service as well as referral of all eligible vulnerable CALHIV to OVC services;
  • Support assigned health facilities with early identification, tracking and return to care of defaulting and lost to follow up clients.
  • Case managers will provide support to HIV positive pregnant women and mother-infant pair actively to ensure all appointments are kept and women are supported through delivery and PMTCT outcome is determined.
  • Maintains a log of all assigned clients contacts (descriptive addresses and phone numbers), and keep comprehensive records of clinic appointments and interventions carried out to support them on retention.
  • Support health facility and community management of infected children and their families across supported sites.
  • Support documentation of interventions in adherence service support monitoring and tracking tools for general and priority (pregnant women, Adolescent and men) population.
  • Account for all clients assigned to them for follow up whether in clinical platforms (community) or the facility;

 Sensitize clients on the differentiated models of care: MMD 3 and 6 dispensing, Community Pharmacy refills, Home refills and Community ART Groups as options available to stable clients.

  • Maintain confidentiality, respect privacy, and preserve the clients’ routine and independence as much as possible.
  • Operationalizes adherence support schedule on the adherence support calendar for test and start clients

Required Qualifications:

  • Diploma in Nursing, Midwifery, CHO, CHEW, JCHEW, Psychology, social work, or any related discipline
  • Experience in HIV/AIDS services &/or a PLHIV is an added advantage
  • Possession of an android device (Phone &/tablet) is mandatory • Knowledge of Computer (MS Word/Excel) is also an added advantage
