Main objective and responsibility of the position
The Outreach Nurse supports the implementation of outreach activities within the Sokoto Noma and Nutrition Project, working closely with Community Health Workers (CHEWs) and outreach teams delivering communitybased health and nutrition services, including ATFC and TSFP programmes.
The role contributes to the early detection, follow-up, and referral of patients, while ensuring the quality implementation of outreach activities in line with MSF principles, protocols, and standards.
General Accountabilities:
- Knowing, promoting, implementing and following up the universal hygiene standards/ precautions, bio-hazard prevention and infection control, security rules and other protocols and procedures in the medical premises and ensuring high standards of hygiene of his/her working environment. Respecting medical secrets and confidentiality at all times.
- Organizing and carrying out care and treatments according to medical prescriptions, and assisting during consultations, daily rounds and other medical procedures. Participating in health education of the patient (and family) when necessary.
- Identifying Sexually Gender Based Violence victims and referring them to the medical team so they can receive the necessary treatment. Knowing and is aware of the importance and appropriate use of the Post Exposure Prophylaxis
(PEP) Kit
- Ensuring patients are properly received and installed and that those with lack of autonomy are assisted, especially regarding their feeding, personal hygiene, movements and comfort. For ER and OPD nurse, ensuring triage of patients in waiting areas, wards or during emergencies, detecting the priority acute/emergency cases, carrying out first aid care and referring them to the doctor when necessary
- Sending material to be sterilized and recuperates it from sterilization
- Carrying out admission, surveillance and follow-up of patients (i.e. assessment of the evolution of their state of health and the identification of any emergency situation or deterioration
- Carrying and supervising administrative procedures and documents (fill in patient’s files, forms, consumptions, statistics, registers, health files, etc.), ensuring an appropriate written/oral handover, and reporting any problematic situations and cases that may arise. Participating in data collection and keeping doctors/supervisors informed .
- Participating in the department-related pharmacy and medical equipment control and maintenance (quality and its functioning, storage conditions, follow-up of expired drugs inventories, stock takes of medicines and material at every team changeover ensuring no material is taken out of nursing area/wards without prior authorisation, etc.)
- Perform cleaning and minor maintenance for biomedical equipment used. Fol-lowing the user manual and protocols and alert supervisor in case of malfunctioning.
- Supervising and training nurse-aids, nutritional assistants, cleaners and other related staff under his/her supervision, ensuring teamwork and adherence to all appropriate protocols and procedures.
Context Specific Accountabilities: Support to Outreach Activities
- Assist in the implementation of outreach activities conducted under the Noma and Nutrition Project.
- Support outreach teams during ATFC and TSFP service delivery to ensure activities follow MSF protocols and guidelines.
- Participate in community outreach sessions aimed at improving access to health and nutrition services.
- Support the identification and referral of patients requiring additional care according to MSF referral pathways.
- Contribute to strengthening links between community structures and MSF-supported health services. Support to Community Health Workers and Outreach Staff
- Assist Community Health Extension Workers (CHEWs), nutrition assistants, and outreach staff in the correct implementation of screening and follow-up activities.
- Provide practical guidance during outreach activities on MUAC measurement, anthropometric assessment, and patient follow-up procedures.
- Facilitate on-the-job coaching and refresher trainings as requested by the Nursing Team Supervisor
- Help ensure that community health workers follow MSF protocols during outreach activities.
Clinical Support and Quality of Care
- Support outreach teams in the screening and monitoring of children aged 6–59 months for malnutrition according to MSF protocols.
- Assist in ensuring correct application of admission, follow-up, referral, and discharge criteria in ATFC and TSFP services.
- Support caregivers by providing basic counselling on nutrition, hygiene, treatment adherence, and follow-up visits.
- Assist medical teams when needed to ensure continuity of care during outreach activities. Community Engagement
- Participate in community awareness and health promotion activities organized by the outreach team.
- Support engagement with community leaders and focal persons to improve access to MSF services.
- Encourage caregivers to seek care early and adhere to follow-up appointments for nutrition services. Therapeutic Nutrition and Supply Monitoring
- Support outreach teams in the correct distribution of Ready-to-Use Therapeutic Food (RUTF) and supplementary nutrition products according to MSF protocols.
- Assist in monitoring the use and distribution of nutritional products during outreach activities.
- Report any irregularities, stock issues, or misuse of supplies to the Nursing Team Supervisor. Data Collection and Documentation
- Support data collection and patient documentation, ensuring completeness and accuracy of patient files, registers, and reporting tools.
- Support the completion of screening forms, referral documents, and follow-up records.
- Assist in compiling routine activity data and submitting reports to supervisors.
- Contribute to maintaining reliable and up-to-date data to support program monitoring.
Infection Prevention and Control
- Apply infection prevention and control measures during outreach activities according to MSF protocols.
- Ensure appropriate hygiene practices are followed by staff and caregivers during service delivery.
- Promote safe handling of medical equipment and disposal of medical waste. Teamwork and Collaboration
- Assist supervisors in monitoring the quality of care and adherence to MSF standards within outreach nutrition activities.
- Work collaboratively with MSF multidisciplinary teams including medical staff, nutrition teams, health promoters, and data officers.
- Maintain effective communication with supervisors and report any operational challenges encountered during outreach activities.
- Participate in team meetings, training sessions, and program discussions as requested.
REQUIREMENTS (QUALIFICATIONS):
Education Bachelor’s degree or diploma in nursing (essential).
Experience Minimum 2 years of clinical nursing experience (essential).
Experience in nutrition programs, community health, or humanitarian settings (desirable)
Languages Fluent Hausa and English (essential).
Knowledge Essential computer literacy (word, excel and internet)
