Community Health Nurse at Candlelight Foundation for Children with Special Needs

Candlelight Foundation for Children with Special Needs is a non-profit organization that is passionately committed to empowering children with special needs, such as Autism, Dyslexia, and ADHD. The foundation provides support, resources, and opportunities for these children to thrive. As the organization seeks to extend its reach, it is actively looking for a dedicated Program Associate who is a resident of Lagos, even though the role is hybrid/remote, to join its team.

Responsibilities

  • Provide basic nursing and community health services to clients in their homes and within the nursing home environment.
  • Monitor clients’ general health and identify changes requiring clinical attention.
  • Conduct routine health checks and maintain accurate client records.
  • Support medication adherence and report concerns regarding prescribed medications to the appropriate nurse or clinician.
  • Provide health education to clients and families on hygiene, nutrition, medication compliance, prevention of illness, and healthy living.
  • Support elderly clients and individuals living with chronic health conditions.
  • Conduct follow-up visits and report clients’ progress to the supervising nurse.
  • Assist with care coordination and appropriate referrals when necessary.
  • Promote infection prevention and safe practices within clients’ homes.
  • Support family members and caregivers with appropriate health and care information.
  • Encourage independence, dignity, and quality of life among residents and clients.
  • Participate in community health outreach and wellness activities where required.
  • Maintain proper documentation of visits, observations, interventions, and referrals.
  • Work closely with Registered Nurses, caregivers, families, and other healthcare professionals.
  • Perform other duties assigned by management.

Requirements

  • Recognised qualification in Community Health Nursing.
  • Valid professional registration/licence where applicable.
  • Experience in community health, elderly care, home care, or primary healthcare is an advantage.
  • Excellent communication and interpersonal skills.
  • Compassionate and patient-centred approach.
  • Ability to work effectively with elderly clients, families, and caregivers.
  • Good documentation and reporting skills.
  • Ability to work independently and as part of a care team.
  • Willingness to travel for home visits where required.
  • Willingness to work flexible hours, including weekends and public holidays.
