Case Managers (Nurse) At Paragone Int’l Health & Support Initiative (PIHSI)


Paragone Int’l Health & Support Initiative (PIHSI) is a Key Population Community Based Organization that promotes the rights and welfare of KPs’ by promoting and supporting a wide range of community development, education, sustainability and self actualization especially amongst FSWs in Nigeria. Paragone believes in the science of saving lives and at the grassroots level has played supportive role and showcased interest in advancing the collective goals of communities members. Paragone has its head office at Awka, Anambra State Nigeria and is implementing in the current GF HIV-NAHI.

Minimum Requirements

  • MUST at least be a Registered Nurse or JCHEW or SCHEW or its equivalent.
  • Support clients to access all recommended HIV care, treatment and support services. 
  • Ensure enrolment and initiation of all HIV positive Clients 
  • Ensure regular monitoring of treatment adherence, tracking defaulting/ lost to follow up clients and facilitate their return to care in collaboration with the dedicated tracking and retention volunteers. 
  • Provide treatment education, adherence counselling and peer support to assigned clients at health facilities and community following the adherence support schedule. 
  • Provide treatment support services (TSS) including clinic appointment reminders and pre-emptive tracking to assigned clients. 
  • Facilitate active referrals of clients to treatment through escort services. 
  • Report and refer clients with adverse drug reaction appropriately 
  • Ensure provision of VL test as per schedule and documentation of results for assigned clients 
  • Maintain a log of all assigned clients contacts (descriptive addresses and phone numbers) and keep comprehensive records of clinic appointments and interventions carried out to support them on retention. 
  • Support documentation of interventions in adherence services, monitoring and tracking tools for general and priority population.
  • Sensitize clients on the differentiated refills for stable clients. 
  • Maintain confidentiality, respect privacy, and preserve the clients’ routine and independence as much as possible. 
  • An experience working with KP communities will be an advantage.

Skills & Attributes  

  • Monitoring blood pressure of clients  
  • Conducting health checks on patients
  • Taking temperatures 
  • Monitoring weight and conducting BMI
  • Processing and taking to lab (OSS) blood samples in a cold chain  Administering medication to client and record them in national tool
  • Must be resident in the state of assignment. 
  • Should be able to communicate in the local language of the community 
  • Knowledgeable on Microsoft Suite 
  • Preferably have an Android Phone and knowledgeable in the use of Android Phones  

Community Facilitators

Minimum Requirements

  • Mobilize community in implementation of the project activities
  • Creates awareness and facilitates project activities  
  • Facilitates in the identification and prioritization of activities sites
  • Ensure participants of the project intervention package are identified and selected in a participatory manner and full acknowledgement of the whole community;
  • Facilitates community meetings and community action plan (CAP) preparation and ensure that the participation of each beneficiary.
  • In collaboration with field supervisor, support in training need identification and provides effective trainings to target community members;
  • Work with community leaders and key staff and acts as a catalyst in initiating action and generating enthusiasm for change in communities by performing demonstrations and organizing and leading sessions;
  • Organizes and facilitates Information Education and Communication (IEC) sessions with community members.

  • Communication skill, good human relations, willingness to reside in rural areas;
  • Firm belief in teamwork, gender equality, conceptual understanding of participatory approach and sustainable development.
  • Familiarity in community mobilization and supporting community structures
  • Respect, accountability, courage, excellence, stress tolerance, information monitoring, developing teams, interpersonal skills, coaching, problem solving, planning and organizing
  • The Community Facilitators is expected to establish open relationship with the community in order to facilitate and encourage active participation in overall development activities and internally, with Supervisors, project, and other concerned staff.
  • The Community Facilitators lives among the community. She/he travels long distances.
  • Demonstrated skills in partnership building and interpersonal communication
  • Excellent and demonstrated skills in computer Microsoft office applications: Word, Excel, PowerPoint and outlooks, emails
  • Knowledge of local language and working with local community
  • Good level of reporting writing skills
  • Participatory facilitation skills and experience
